Medicaid
Rocky Mountain Health Plans (RMHP) has served Western Colorado since 1974. We work closely with the State of Colorado and with local private and government organizations to provide or coordinate the delivery of a full spectrum of publicly funded Medicaid benefits.
Rocky Mountain Health Plans serves Medicaid clients living in these counties:
- Delta
- Mesa
- Moffat
- Montrose
- Ouray
- Rio Blanco
- San Miguel
RMHP arranges for Medicaid clients to receive benefits covered by the Colorado Medicaid program. All providers in RMHP's network are available to Colorado Medicaid clients. RMHP also provides customer service, case management, disease management, and quality monitoring and improvement services.
RMHP doctors give special check-ups and follow-up services to children through age 21 who are covered by the Medicaid EPSDT program. RMHP nurses work closely with members who have diabetes, asthma, heart disease, and other chronic illnesses to help them manage their health status.
Colorado Medicaid offers some additional services that are not covered through Rocky Mountain Health Plans. RMHP Customer Service representatives will direct you to the right places to get dental care, mental health care, and substance abuse treatment through the Special Connections program.
How to Join RMHP Medicaid
You must be eligible for Medicaid before you can enroll in RMHP Medicaid. If you don't know if you are eligible for Medicaid, contact your county department of human services.
If your department of human services says you are eligible, you may choose to enroll in RMHP Medicaid. To do this, call Health Colorado toll-free at 888-367-6557. Tell them you want to join Rocky Mountain Health Plans.
You may call RMHP at 970-243-7050 Monday through Friday from 8:00 a.m. to 5:00 p.m. with questions about RMHP Medicaid.

CHP+
The Child Health Plan Plus (CHP+) helps families who cannot afford health insurance and who do not qualify for Medicaid. CHP+ enrolls children in a health benefits plan at a low cost.
When you are enrolled in CHP+, your children can go to the doctor or get medication whenever they need it. You can take your child to any of the RMHP participating providers who practice in Delta, Mesa, or Montrose counties.
CHP+ covers outpatient and inpatient services. Mental health services are available through RMHP.
If a member covered by CHP+ becomes pregnant, she will receive all her pregnancy, delivery, and follow-up care from RMHP providers and hospitals.
CHP+ covers dental care. It is available through a separate dental netowrk and is not covered by RMHP. Our Customer Service Department will tell you how to contact the person you need to help you get dental care.
How to Join CHP+
To ask about enrolling your child, call CHP+ toll free 800-359-1991, Monday through Friday, from 8:00 am to 6:00 pm.
Those eligible for CHP+ must be children who are:
- age 18 or younger
- not eligible for Medicaid
- living in a financially qualified family
- Colorado residents
- US citizens or permanent US residents who entered the US before August 22, 1996
If your child is eligible, he or she will become a member of RMHP CHP+. RMHP will send you information about your child's benefits. You may also call RMHP Customer Service at 970-243-7050 to get more information.
